EARTHHOUSE MEMBERSHIP --- Show your support for the idea of a Community Center that the Pagan Community can call it’s own.

• Only $20.00 per year for Associate (non-voting) Membership. Voting eligibility is contingent on fulfilling participation requirements. (This Festival counts towards those.)

• Membership is renewable July 1, and is valid until the next June 30

• The potential to vote in the business meetings

• Voting Members are eligible to run for office

• The warm, inner glow of helping to further the building of community and an actual community center in one of the largest Pagan hotspots in the nation.

T-Shirts with this years LOGO design are available for $10.00 each if ordered with your registration. They will be available at The Gather for $15.00 each.

THE LEGAL STUFF

These are your agreements with Earth House Project, Inc.

They become effective when you sign this form

You will be required to sign a hard copy upon arrival at The Gather

RELEASE OF LIABILITY

     I agree that my participation in the Earth House Project Inc. or the Earth House Midsummer Gather, shall be at my own risk, and will hold them harmless for conditions, injuries, or damages, known or unknown, which I might incur or aggravate as a result of my participation.

     I further agree by signature or submission of this form, and for due and adequate consideration, receipt for which is hereby acknowledged, do hereby release and indemnify the Earth House Project Inc. or the Earth House Midsummer Gather, and officers, members, volunteers, representatives, agents and employees of that organization their heirs and assigns (hereinafter jointly referred to as "Released Parties") from all claims of any nature for injury to myself and also for any minor child over whom I have care, guardianship, custody or control, whether caused by the negligence of any Released Parties or not. This release includes permission and release of liability for any minor child in my custody or control to participate in Earth House Project Inc. and the Earth House Midsummer Gather.

      I further agree to indemnify each, any and all Released Parties for any judgment or settlement amount, costs and/or expenses which result from any action brought against any Released Parties as a result of injuries sustained by myself or any minor child over whom I have care, guardianship, custody or control. I understand that this Release and Waiver is intended to protect the Released Parties from liability in order that the possible threat of personal liability might not inhibit or prevent Released Parties from offering the this event provided by Earth House Project Inc. or the Earth House Midsummer Gather, to adults or minor children.

     I agree that if I bring any personal property to the Earth House Midsummer Gather, and I store or leave any personal property at the Earth House Midsummer Gather, I will do so at my own risk and that none of the Released Parties shall have any liability in the event of loss, damage, unauthorized use (by any person other than a Released Party), theft, or injury resulting from the personal property. I agree that if any provision of this release shall be held to be void, all other provisions shall still apply.

     I certify that I am at least 18 years of age, am under no legal disability that would prevent be from entering into a legal agreement and have read and understood the above agreement.

PHOTO RELEASE

     Unless specified otherwise, I hereby grant the Earth House Project, Inc. (hereinafter "Earth House"), its representatives, employees and assigns the right to take photographs of me and my property in connection with the Earth House Midsummer Gather. I authorize Earth House, its assigns and transferees to copyright, use and publish the same in print and electronically. i agree that Earth House may use such photos of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising and Web content.

By submitting this form as Participant and/or Parent/Guardian of any minors attending with you, you agree to:

1. Abide by the rules of the Midsummer Gather, AND

2. Agree to the terms of the Release of Liability, AND

3. Agree to the Photo Release (unless otherwise specified)
